Waiver of Liability

Instructions and Helpful Hints:

If the USER is under 18, a GUARDIAN must Sign/Print Name on the bottom line under the USER's name and signature. It's important that the USER's GUARDIAN be aware of our policies and rules. If this is left blank, we cannot accept the USER's waiver.

Helmets are provided free of charge when requested by the USER or GUARDIAN. Our Helmet waiver is located near the bottom of the first page, and should be read carefully. RockQuest and its employees encourage the use of an UIAA approved climbing helmet for all participants.
